The purpose of this form is to request lodging availability for accommodations at Lifestyle Holidays Vacation Club Resort - a fabulous All-Inclusive Luxury Caribbean Resort in Puerto Plata, Dominican Republic on the beautiful Cofresi Beach. The resort offers luxury villa rentals ranging from 6-bedroom / 6 bathroom to 3 bedroom / 3 bathroom , as well as penthouses, suites and deluxe hotel rooms and suites. Rates are based on a one week stay which is typically from Sunday to Sunday, but can begin and end on any day of the week, and can be prorated for a partial week upon request . The Accommodation Rates are PER UNIT and NOT PER DAY OR PER PERSON. The Accommodation rates must be paid to us in full, in order to receive confirmation, , Accommodation Fees are Non-Refundable, so you will want to check your flights before completing your reservation. RESORT FEES - A per person / per night resort fee will be added to all adults.  The additional resort fee is $5 per person / per night for all accommodations, except the Villas and Presidential Suites, which are $10 per person / per night.  

KOSHER fees are: $99 per person/per night / Half price for children 3-11 and children under 3 are free. All Meals are eaten at Blues Restaurant, near Presidential Suites.
